Eligible physicians to join our growing Hospice and In-Home

Dedicated hospice team serving patients for 40 years. ·Inpatient
Hospice Unit at main campus- 7 bed unit · Free standing 16 bed
hospice currently under construction just minutes from main campus
· Physicians support nursing and interdisciplinary staff by
providing oversight for hospice and in-home palliative care ·
Desirable schedule Monday through Friday schedule ·Service Line
supported by dedicated: o Advanced Practitioners o Licensed
Clinical Social Workers o Chaplains o Designated nurse staffing ·
Growing program with opportunity to work with residents and fellows
Working together with Physicians like you: · Board-Certified or
Board-Eligible in Hospice & Palliative Medicine (HPM) ·Active
Georgia license or ability to obtain unrestricted license
·Experience providing care across settings including inpatient
consults, hospice inpatient units, home hospice, and outpatient
palliative care · Passion for quality and patient care · Strong
communication skills and comfort with goals-of-care discussions,
symptom management, and end-of-life care planning ·Functions well
in a team environment, contributing to and supporting
organizational goals and initiatives · Physicians holding legacy or
time-limited HPM certification remain fully acceptable, provided
certification is maintained per governing board requirements About
